The following forms must be signed and returned by October 31, 2026 to pageant@humblerodeo.com by for your registration to be complete - No Exceptions. Forms are found below.
- Contestant Profile Form—Complete as neatly as possible and attach a picture (Picture required)
- Contestant Contract—Contestant signs and dates. If contestant is under 18 years of age, parent must also sign and date.
- Essay

Medical & Security Form—Fill out and sign by contestant. If contestant is under 18 years of age, parent must also sign and date.
